Who controls your information

The organization that works with you (for example, a reentry, housing or recovery program) decides what is recorded about you and who on its team can see it. We provide and run the platform for them. Questions about your record are best answered by that organization; you can also contact us as described below.

What we collect

• Staff accounts: name, work email and the organization each person belongs to, so they can sign in.
• Participant records the organization keeps, or that you add in your own Ascend app: contact details, the forms you complete with them, your plan, needs, documents, appointments and messages.
• Mobile numbers and whether you agreed to receive texts, with when and how you agreed.
• Requests sent through the contact form on this website: your name, organization, email and message.
• Security logs recording who accessed or changed a record, kept so access can be reviewed.

How we use it

Only to provide Ascend to the organizations that use it: to show their staff the records they are allowed to see, to send the emails and texts they send through Ascend, to keep the service secure, and to answer requests sent through this website. We do not sell personal information and we do not use it for advertising.

Text messages

Mobile numbers are used only to send the service messages described in our text message terms. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent are excluded from every other category of sharing in this policy and are not shared with any third party.

Sharing

Information is shared only as the organization directs (for example, a referral you agreed to, to a partner organization) and with the service providers that run the platform for us, such as Microsoft Azure for hosting, email and text delivery. They process it only to provide those services.

Security and where it is stored

Ascend is hosted in Microsoft Azure in the United States. Information is encrypted in transit and at rest, each organization's data is kept in its own database, and access is limited by role and recorded.

Your choices

You can ask the organization you work with to see or correct your record, view much of it yourself in your own Ascend app, and reply STOP to any text to stop receiving them.
